How UK-Trained Doctors Can Work in Sweden

UK doctors moving to Sweden after Brexit do not all follow the same licensing route. Socialstyrelsen, Sweden’s National Board of Health and Welfare, currently uses the date on the doctor’s UK medical diploma as the key dividing line: a UK diploma issued before 1 January 2021 may be assessed through the EU/EEA route, while a diploma issued on or after 1 January 2021 is handled as a qualification from outside the EU/EEA. Immigration is a separate question. A British citizen moving now will normally need a Swedish residence or work permit unless another status or citizenship gives them a right to live and work in Sweden.

This guide explains that post-Brexit split, the steps attached to each route, the Swedish-language requirement, and how licensing must be coordinated with immigration and job applications.

How Brexit changed the route for UK-trained doctors

The United Kingdom is no longer an EU/EEA country, but Sweden does not place every British medical diploma into the non-EU route. Socialstyrelsen’s current guidance for people educated in the United Kingdom states that a diploma issued before 1 January 2021 can be assessed on the same basis as a qualification from an EU member state. The authority links this treatment to an addition to Annex V of Directive 2005/36/EC.

The relevant date is the diploma’s issue date, not the date when the doctor started medical school, became a British citizen, obtained GMC registration, or decided to move. If the wording or date on your document is unusual, obtain written guidance from Socialstyrelsen before choosing an application category.

Route 1: UK diploma issued before 1 January 2021

If your primary UK medical diploma was issued no later than 31 December 2020, Socialstyrelsen says that a first-time applicant should apply in the category for education completed within the EU or EEA. This is a recognition route; it is not an automatic transfer of GMC registration. Sweden still checks the qualification, documents, professional status, language evidence, identity, and other licensing conditions.

Documents normally required

According to Socialstyrelsen’s current application page for doctors educated within the EU/EEA, a typical application includes:

  • a valid identity document;
  • the medical diploma;
  • a Certificate of Good Standing or Certificate of Current Professional Status that is no more than three months old;
  • a Certificate of Conformity showing that the training meets the relevant EU Directive requirements;
  • accepted evidence of Swedish, Danish, or Norwegian proficiency;
  • evidence of a name change, where applicable; and
  • additional specialist documents if specialist recognition is requested at the same time and the route permits it.

UK documents written in English do not normally require translation under the EU/EEA application instructions. Documents in another language must meet Socialstyrelsen’s translation rules. Do not send a university letter as a substitute for a document that must be issued by a competent professional authority.

Doctors who hold or previously held GMC registration can ask the General Medical Council to send a Certificate of Good Standing directly to an overseas medical regulator. The GMC says these certificates are valid for three months, so request one close enough to the Swedish application date. Confirm with Socialstyrelsen which authority must issue every route-specific document, especially the Certificate of Conformity.

If Socialstyrelsen assessed you under an older practice

A doctor with a pre-2021 British diploma may already have received a Swedish assessment under an earlier interpretation of the post-Brexit rules. Socialstyrelsen says that an existing decision remains valid, but there may now be another way to assess the qualification. Contact the authority with the case number and ask for individual guidance; do not submit a duplicate application merely because the public guidance changed.

Possible compensatory measures

Applying through the EU/EEA category does not guarantee immediate recognition in every case. If Socialstyrelsen identifies substantial differences, it may issue a decision requiring a compensatory measure, such as an adaptation period or aptitude test. Follow the exact decision in your case rather than assuming that the non-EU kunskapsprovet automatically applies.

Route 2: UK diploma issued on or after 1 January 2021

A UK medical diploma issued from 1 January 2021 is treated as education from outside the EU/EEA. Socialstyrelsen currently describes two main ways to obtain a Swedish licence: its five-step authority route or an eligible complementary programme at a Swedish university. Admission to a programme is separate from Socialstyrelsen’s assessment and is not guaranteed.

The five-step Socialstyrelsen route

  1. Have the medical education assessed. Socialstyrelsen compares its level, length, and content with Swedish medical education. Swedish is not required for this first assessment, and the applicant does not need to live in Sweden at this stage.
  2. Pass the proficiency test. The kunskapsprovet assesses theoretical and practical competence. Preparation and examination details should be checked on the current official test pages.
  3. Complete the course in Swedish laws and regulations. This online course addresses the legal and regulatory framework governing Swedish healthcare.
  4. Complete clinical training. The applicant undergoes the practical service specified for the route and needs the required authorisation for that work.
  5. Apply for the licence. At the final stage, Socialstyrelsen checks completion of the requirements, language evidence, identity number, and the other documents required for a licence.

Socialstyrelsen says that this authority route usually takes two to four years, but that is general guidance rather than a guaranteed processing time. The actual duration depends on the initial decision, Swedish proficiency, test scheduling and results, access to clinical training, document completion, and the applicant’s circumstances.

The complementary-programme route

Instead of completing the five authority steps, an eligible doctor may apply to a Swedish university’s complementary programme for doctors educated outside the EU/EEA. Places are limited, the university decides its admission requirements, and Swedish is required. Socialstyrelsen describes the overall sequence as complementary training, medical internship (AT), and the final licence application. Its general estimate is one to three years, but admission waiting time and individual requirements can extend the total journey.

Do not choose between these routes solely by comparing the published estimates. Consider eligibility, Swedish level, admission cycles, examination readiness, finances, permission to live in Sweden, and whether you can realistically secure the required clinical stage.

Swedish language requirements for UK doctors moving to Sweden

English clinical experience does not replace Sweden’s language requirement. Socialstyrelsen requires accepted evidence of sufficient Swedish, Danish, or Norwegian proficiency before it can issue the licence. Its current pages list options including Swedish at CEFR C1, Swedish 3 or Swedish as a Second Language 3, a Scandinavian-language qualification that gives access to higher education, or a documented employer assessment at an equivalent level when the authority’s conditions are met.

Licensing evidence is only one part of readiness. A doctor must also understand patients, document safely, discuss risk and consent, communicate during handovers, and work with Swedish colleagues. Starting early is particularly important for post-2020 graduates because the proficiency test and later licensing stages are conducted in Swedish.

Does a British doctor need a Swedish work permit?

Usually, yes, if the doctor is a British citizen moving to Sweden now and has no other basis for residence. The Swedish Migration Agency’s guidance for British citizens says British citizens no longer have EU/EEA right of residence and normally need the residence permit that corresponds to what they plan to do in Sweden.

Important exceptions and alternatives may include:

  • valid residence status under the EU-UK Withdrawal Agreement;
  • another EU/EEA citizenship and a qualifying right of residence;
  • a residence card or permit based on a family relationship;
  • another existing Swedish residence status; or
  • a permit category based on employment, studies, research, or another lawful purpose.

For a standard employee work permit, the employer and employment must meet current conditions concerning the contract, salary, insurance, and employment terms. Rules changed on 1 June 2026, and the Migration Agency lists a specific salary-threshold exemption for certain foreign-qualified doctors, dentists, and nurses who are in the process of obtaining Swedish professional certification. This does not remove the other permit requirements or mean that every healthcare job qualifies.

A licence and a work permit solve different legal problems. Socialstyrelsen decides whether you may practise in a regulated healthcare profession; Migrationsverket decides whether you may live and work in Sweden under immigration law. Receiving one does not automatically grant the other.

Can you apply for jobs before receiving the licence?

You can research employers, build Swedish proficiency, prepare documents, and discuss future positions before the licence is issued. Whether you may perform medical work before full läkarlegitimation depends on the position, your stage in the process, and whether a valid särskilt förordnande (special authorisation) or another lawful arrangement applies. Do not describe yourself as holding a Swedish medical licence until Socialstyrelsen has issued it.

When approaching a Swedish employer, state your status precisely:

  • the country and date of your primary medical diploma;
  • your current GMC registration and licence status;
  • the Swedish application route you are using;
  • the Socialstyrelsen decision or step already completed;
  • your current Swedish level and planned evidence; and
  • whether you need employer support for a work permit.

What if you are already a UK specialist?

A UK specialist or GP qualification does not automatically become a Swedish specialist certificate. Basic medical licensing and specialist recognition are related but separate decisions. A doctor whose UK diploma falls within the pre-2021 route may be able to submit specialist evidence under the relevant EU/EEA process, while a doctor in the outside-EU/EEA route must first obtain the Swedish medical licence and then follow the applicable specialist rules.

Because qualification dates, specialty titles, GMC register status, training history, and previous recognition can change the evidence required, specialists should ask Socialstyrelsen to confirm the correct route in writing. An employer may value NHS experience, but employer recognition of experience is not the same as formal recognition of a Swedish protected specialist title.

Common post-Brexit mistakes

  1. Using nationality to choose the licensing route. The medical qualification and its issue date determine the route described in Socialstyrelsen’s UK guidance; citizenship is central to immigration.
  2. Assuming all post-Brexit applicants must take the same exam. A pre-2021 UK diploma can enter the EU/EEA application category. Individual assessment may still produce additional requirements.
  3. Treating GMC registration as portable. GMC status is valuable evidence, but Sweden issues its own licence.
  4. Requesting time-limited documents too early. A Certificate of Good Standing must be current when submitted; coordinate it with the rest of the application.
  5. Waiting to learn Swedish. Language preparation often becomes the longest controllable part of the project, especially for the outside-EU/EEA route.
  6. Confusing a licence with immigration permission. Confirm the professional and migration tracks separately before agreeing on a start date.
  7. Assuming specialist recognition is automatic. Ask for a separate assessment of the specialty and do not use a Swedish specialist title prematurely.

A practical sequence from the UK to Sweden

  1. Check the exact issue date on your primary medical diploma.
  2. Confirm your route on Socialstyrelsen’s UK page. Seek individual advice if the document date, place of education, or previous assessment creates uncertainty.
  3. Map the required documents. Note who must issue each item, whether it must be sent directly, and how long it remains valid.
  4. Begin Swedish immediately. Plan both formal proof and practical medical communication.
  5. Submit the correct first application. Pre-2021 diplomas normally use the EU/EEA category; post-2020 diplomas use the outside-EU/EEA category.
  6. Follow only the steps in your decision. Do not copy another doctor’s route if the qualification history differs.
  7. Prepare the employment track. Research regions and employers, present your licensing stage accurately, and discuss permit sponsorship when relevant.
  8. Coordinate licence, permit, and start date. Obtain the professional authorisation and immigration permission required for the actual position before beginning work.

Frequently asked questions

Do all UK doctors have to take the Swedish kunskapsprov after Brexit?

No. Socialstyrelsen currently says that a UK medical diploma issued before 1 January 2021 can be assessed through the EU/EEA route. A diploma issued on or after that date is handled through the outside-EU/EEA route, where the five-step option includes the proficiency test. An individual EU/EEA assessment can still result in compensatory measures.

Does a British passport decide which medical-licensing route I use?

No. Under the current UK-specific guidance, the diploma’s issue date determines whether a UK qualification is submitted through the EU/EEA or outside-EU/EEA category. Citizenship and residence status determine whether you need immigration permission.

Can I transfer my GMC registration directly to Sweden?

No. You must apply to Socialstyrelsen for a Swedish licence. GMC registration and a current Certificate of Good Standing may form part of the evidence, but they do not themselves authorise medical practice in Sweden.

Do UK-trained doctors need C1 Swedish?

Doctors must prove sufficient Swedish, Danish, or Norwegian using one of Socialstyrelsen’s accepted methods. CEFR C1 Swedish is one option. Swedish 3, qualifying Scandinavian education, and a properly documented healthcare-provider assessment may also be accepted when their conditions are satisfied.

Can I apply to Socialstyrelsen while I am still living in the UK?

Yes. Socialstyrelsen does not require residence in Sweden simply to submit a qualification application, and the outside-EU/EEA education assessment can begin without Swedish. Later steps, the final licence, clinical work, and relocation may require a coordination number, Swedish proficiency, presence in Sweden, and the correct residence or work permission.

Does a British citizen always need a work permit?

Not always. A British citizen moving now normally needs an appropriate Swedish permit, but Withdrawal Agreement residence status, another EU/EEA citizenship, a family-based right, or another valid status may change the answer. Confirm the current category with Migrationsverket before planning a start date.
