How to Work as an Underläkare in Sweden

Underläkare jobs in Sweden can be open to medical students, doctors waiting for Swedish legitimation and already licensed doctors, but the job title alone does not establish who may work or under what authority. Your education, licence status, employer and workplace determine whether you need a time-limited särskilt förordnande, or special appointment, before starting.

This guide explains the practical route: identify your legal category, confirm who must obtain the decision, target suitable vacancies, prepare a Swedish application and examine supervision before accepting an offer. It also separates getting an underläkare job from later questions about BT or ST credit. Use the Swedish medical job-title glossary for a broader comparison of underläkare, legitimerad läkare, ST-läkare and specialist roles.

Underläkare jobs in Sweden at a glance

Underläkare is an employment label used in different situations. It can describe a temporary post held by a medical student after sufficient study, a doctor in a pre-licence stage or a licensed doctor working before entry into specialist training. The decisive question is therefore not simply, “Can I call myself an underläkare?” It is, “What legal basis lets me perform this doctor’s work in this workplace during these dates?”

Swedish law says that a person may practise as a doctor only with a Swedish medical licence or a special appointment. The protected professional title läkare has separate rules. A special appointment permits specified work for a specified post and period; it does not by itself give the unrestricted right to use the protected title. The employer must classify the post correctly and verify the authority required before work begins.

The advert title is not the authority

An advert headed “underläkare” does not override licensing law. Ask the employer to confirm in writing whether the post requires legitimation, a special appointment or another recognised basis, and who is responsible for obtaining any decision.

A sensible sequence is: document your status, read the eligibility line in the advert, discuss the planned duties with the recruiting manager, and verify the formal decision route before signing or relocating. Immigration permission is separate from professional authority. EU/EEA citizens generally have the right to work under EU free-movement rules, while many non-EU/EEA applicants need a work permit that meets the current Migration Agency conditions.

Who can work as an underläkare?

There is no single application route for every candidate. The categories below are a practical starting point, not a substitute for an employer’s or Socialstyrelsen’s written assessment. A doctor who already qualifies to apply for Swedish legitimation should normally use that licensing route rather than seek a special appointment.

Common applicant categories and the question each must resolve
ApplicantPossible basisEvidence to prepareKey limit
Swedish medical studentSpecial appointment for an eligible temporary post after at least 10 completed termsStudy certificate showing completed courses and examinationsThe pre-degree student route does not permit work in primary care
EU/EEA medical studentSpecial appointment decided by Socialstyrelsen after at least 10 completed termsCompleted-term evidence and accepted Swedish-language evidenceEmployer and dates must match the application and decision
Doctor educated outside the EU/EEALicence, or a special appointment in one of the permitted pre-licence situationsSocialstyrelsen decisions, knowledge-test or practical-service records where relevantThe exact decision-maker depends on the route and whether the provider is public or private
Doctor with Swedish legitimationSwedish medical licenceLicence details and relevant clinical referencesThe post may still have local competence, language and supervision requirements

Since 1 February 2026, the student threshold in the special-appointment rules is at least 10 completed terms. The earlier transition that could allow applications after nine terms ended on 31 January 2026. “Completed” means that the relevant courses and examinations have been passed, not merely that the student has attended the term.

For EU/EEA medical students, Socialstyrelsen also asks for evidence of Swedish ability through one of its accepted alternatives, such as Swedish 3, Swedish as a Second Language 3, equivalent higher-education eligibility or documented C1-level proficiency. Doctors following the EU/EEA or outside-EU/EEA licensing routes should separately check the language evidence required for legitimation. A vacancy may impose an operational language requirement even where the formal route is described differently.

UK qualifications require a date check. Socialstyrelsen directs applicants whose qualification was obtained before 1 January 2021 to the EU/EEA route, while qualifications obtained on or after that date follow the outside-EU/EEA route. Nationality alone is not a reliable way to select the professional-recognition pathway.

Special appointment before Swedish licence

A särskilt förordnande is a temporary authority to practise as a doctor in a particular employment and period without Swedish legitimation. It is not a general permit that follows you from employer to employer. The separate guide explains how working before legitimation with särskilt förordnande fits the wider licensing process.

Who makes the decision depends on the applicant’s situation and the provider. A region may decide certain cases covered by the regulations—for example, some appointments for eligible medical students and some outside-EU/EEA doctors who have reached specified stages in Socialstyrelsen’s route. A private healthcare provider cannot issue the appointment itself, even if it works under a regional contract; it must apply to Socialstyrelsen. Other cases also require an application to Socialstyrelsen by the employer or region.

The application should identify the candidate, workplace, position, dates and planned supervision. Supporting documents vary by category. They can include study certificates, Socialstyrelsen decisions, evidence of passed knowledge-test stages, practical-service information and language evidence. Give the employer complete, legible documents early and ask which authority will decide the case.

Do not start before the authority is in place

Socialstyrelsen does not grant a special appointment retroactively. The requested period must be in the future. Treat the written decision—not an informal assurance or a proposed start date—as the control point for doctor’s work that requires the appointment.

Supervision is not a decorative condition. Socialstyrelsen’s guidance expects support adapted to the person’s education and experience, access to a licensed doctor who can supervise, and colleagues available for instructions. Agree how the supervisor will be contacted during ordinary work, evenings and weekends; how cases will be reviewed; and which decisions require immediate escalation.

What underläkare work involves

The working day depends on specialty, care setting, licence status and local delegation. Typical elements may include taking histories, examining patients, presenting cases, drafting notes, preparing discharge information, joining rounds, ordering or following up investigations when authorised, and discussing treatment plans with a supervisor. An emergency department, psychiatric unit, surgical ward and community service will organise these tasks differently.

Swedish healthcare organisations allocate many tasks according to individual competence, but some tasks are specially regulated. Every healthcare professional also carries personal responsibility for how they perform their work. This means an underläkare should know both the unit’s routine and the boundary of their own competence. “A colleague usually does it” is not a safe basis for accepting a task.

Clarify responsibility before the first shift
AreaAsk the employerSafe working habit
Clinical decisionsWhich cases and decisions require supervisor review?Escalate early when the situation exceeds your competence or authority
DocumentationWhich record system, templates and signing routines apply?Write timely, clear notes and confirm how drafts are reviewed
Medicines and certificatesWhich actions are restricted by law or local policy?Do not assume access to a system means authority to use every function
On-call workIs a licensed doctor on site or immediately reachable?Know the escalation number and backup plan before taking responsibility
Patient communicationWhat interpreting and accessibility support is available?Check understanding; do not use relatives as the default interpreter

Ask for a structured introduction to the ward, the electronic record, prescribing controls, referral routes, incident reporting and local emergency procedures. If the role starts with observation or paired shifts, find out what competence must be demonstrated before responsibilities expand. A useful post should make the path from supported work to greater independence explicit.

Where and when to look for vacancies

Start with the career portals of Sweden’s 21 regions and the websites of hospitals, clinics and private providers. Search Swedish terms such as underläkare, vikarierande underläkare, läkarvikariat and sommarvikariat läkare. Regional pages regularly publish temporary underläkare posts, but the eligibility wording, specialty and closing dates vary.

Summer recruitment can begin months before the holiday period. Departments may also recruit for parental leave, study leave, staffing gaps or a fixed pre-training period. Set alerts where available and check portals consistently. A direct, professional enquiry to a department can be useful, but it should identify your exact licensing stage and availability rather than merely ask whether “any work” exists.

Geographic flexibility can widen the search. Smaller hospitals and northern or rural services may advertise opportunities that differ from university-city competition, although no location guarantees a job. Compare the clinical support, accommodation realities, travel and work-permit implications—not just the likelihood of an interview.

Use the first Swedish doctor job strategy for the broader search across regions, contacts and unadvertised opportunities.

Read every advert closely. “Legitimation required,” “10 completed terms,” “passed knowledge test,” and “C1 Swedish” describe materially different candidate groups. If your status is close but not identical, ask before applying and describe the difference accurately. Never imply that an application for a licence, test or certificate has already been approved.

How to apply for an underläkare job

A strong application makes eligibility easy to assess. Put your Swedish licence status or licensing stage near the top of the CV, then list medical education, relevant clinical experience, Swedish level, current location, work authorisation and availability. Use month-and-year dates, explain gaps briefly, and distinguish supervised placements from employed doctor’s work.

The cover letter should answer three questions: why this department, why you are ready for its patient group, and what supervision or formal decision you require. Connect two or three examples to the advert instead of repeating the CV. Evidence can include managing a comparable caseload, communicating a difficult plan, improving documentation, or recognising when to escalate.

Language evidenceAccepted certificate where required, plus practical evidence that you can communicate in clinical Swedish

Prepare a short Swedish account of your current route: what is complete, what remains, and what an employer would need to do. This prevents confusion during screening. If a special appointment may be necessary, do not present it as automatic. State that the post and your documents must be assessed and that the correct employer or authority must make the decision.

Interview preparation should cover clinical reasoning and workplace communication. Practise presenting a patient concisely, prioritising a busy list, responding to an uncertain instruction and asking for help. Be ready to explain differences between your previous healthcare system and Swedish routines without criticising either system.

Assess the offer before accepting

An offer is only the beginning of due diligence. Request a written job description, workplace, dates, working percentage, schedule, salary, collective-agreement information and named supervisor. Confirm whether nights or weekends are included and what licensed medical support is available then. If relocation is involved, separate confirmed support from suggestions about possible housing.

Ask the employer to name the legal basis for the work. If a special appointment is required, identify who will apply, which documents remain outstanding and what happens if the decision is delayed. The employment contract, professional-authority decision and immigration permission may have different timelines; none should be treated as a substitute for the others.

For supervision, ask about frequency as well as availability. Will there be daily case review, scheduled feedback and an introductory competence assessment? Who covers when the usual supervisor is absent? How is unsafe workload escalated? A vague promise that “someone is always nearby” is weaker than a documented arrangement.

Finally, check how experience will be evidenced. Agree on an employment certificate describing dates, working percentage, setting and duties. If you hope to use the experience in a later BT or ST application, ask whether the required supervisor and manager will be able to certify objectives—but do not assume advance local agreement guarantees Socialstyrelsen credit.

Swedish language and workplace readiness

Formal language evidence and safe workplace communication overlap, but they are not identical. A certificate can satisfy an administrative requirement; an employer must still judge whether you can understand patients, document accurately, receive telephone information, participate in handovers and escalate risk in its clinical environment.

Prepare for the language of uncertainty: “I need to verify this,” “I am concerned about deterioration,” and “Please repeat the dosage.” Learn common abbreviations cautiously and ask how the unit records negative findings, consent, interpreter use and safety-netting. Listening across dialects and speaking under time pressure often need deliberate practice even after advanced general Swedish.

Prepare Swedish for an underläkare post

Our Swedish for healthcare professionals programme connects general Swedish through C1 with live Writing & Speaking, Swedish for Medical Staff and conversation practice. It is designed to strengthen the language used in patient interviews, handovers, journal notes and escalation conversations. Our C1 certificate is accepted by Socialstyrelsen as proof of language proficiency.

Before starting, ask for local learning material and system training. Review the specialty’s common patient information, practise structured handover formats and build a personal list of phrases you must understand instantly. Language readiness includes knowing when communication has failed and using an interpreter or colleague safely.

Underläkare experience before BT or ST

An underläkare post can build Swedish clinical experience, references and familiarity with local systems. It does not automatically become BT or ST time. Socialstyrelsen’s current BT guidance allows certain relevant service completed after the medical degree but before legitimation, under supervision and against BT objectives, to be credited for up to six months. The main clinical manager certifies the service and Socialstyrelsen assesses it in the specialist-competence process.

The detailed guide explains when underläkare experience can affect BT or ST.

If future credit matters, collect evidence while managers and supervisors can still verify it. Keep the contract, schedule, workplace details, supervision arrangement, employment certificate and any objective-based assessments. Ask for accurate descriptions rather than wording designed to promise a result that only the competent authority can decide.

Frequently asked questions

Can a foreign-trained doctor work without Swedish legitimation?

Only when another lawful basis applies, most commonly a special appointment for the specific post and period. Eligibility depends on the doctor’s education, licensing stage, employer and proposed duties. A job offer alone does not authorise doctor’s work.

Can a medical student work as an underläkare in primary care?

Not under the pre-degree student special-appointment route described by Socialstyrelsen. That route requires at least 10 completed terms and excludes primary care before the medical degree. Other applicants must be assessed under the basis that applies to them.

Is an underläkare the same as a resident doctor?

No. Underläkare is a broad employment label used before or outside an ST post. An ST-läkare is a licensed doctor employed in formal specialist training. Read the advert and contract rather than translating the Swedish title into a foreign training grade.

Who applies for a special appointment?

It depends on the category. Regions may decide certain appointments permitted by the regulations. A private provider cannot issue one and must apply to Socialstyrelsen, including when it works under a regional contract. Ask the employer to document the applicable route.

Is C1 Swedish required for an underläkare job?

Socialstyrelsen requires accepted language evidence in specified licensing and special-appointment routes, and C1 is one accepted form. Employers may set language criteria for safe work. Check the legal route and the vacancy instead of assuming one universal requirement for every underläkare post.

Does underläkare work count toward BT?

Not automatically. Relevant post-degree, pre-licence service under supervision may be credited against BT objectives for up to six months. The service must be certified appropriately, and Socialstyrelsen assesses whether the requirements are met.

Can an overseas specialist apply for underläkare posts?

Potentially, if the employer’s eligibility criteria and the lawful basis for work are satisfied. Overseas specialist experience does not automatically confer the Swedish protected specialist title. Keep the employment question separate from recognition of specialist competence.

Next steps for an underläkare application

First, write a one-page status summary covering your education, Swedish licence stage, language evidence, right to work and earliest realistic start date. Second, target adverts that match that status and tailor each application to the unit. Third, verify the legal basis, decision-maker, supervision and duties before accepting.

A good underläkare post should provide more than access to Swedish healthcare. It should offer a lawful appointment, clear limits, reachable supervision, relevant clinical learning and evidence you can use later. Keep every formal decision and employment record, and recheck authority guidance whenever your status, employer or proposed dates change.
